If he is exactly 15 kg you can use the medium size.

If my dog gains weight and goes over 15kg, how should I prescribe rest of the pills?

If it is round 15 kg., this product should be good enough. But it is more than half a kilo heavier then you should consider using the NG Spectra for large dogs.
